Has anyone else encountered similar issues, and if so do you have any recommendations for a fix? I’m pretty comfortable with *nix and tech in general so Terminal commands are welcome. A password manager, digital vault, form filler and secure digital wallet.

I have not (yet) done a full reset of Safari, but I think that’ll be my next step. Last Pass Help recommends installing version 4.6 for macOS Big Sur from their direct download site instead of the app store version. Depending on your environment's change control procedures, you may want to disable auto-updates if end users don't have admin rights.
